House League Registration
Age Groups for the 2012 House League season:
- Atom − 2002, 2003, 2004, 2005 & 2006
- Peewee − 2000 & 2001
Forms required to be completed in full, and can be found on the website:
- OFA form ( 1 copy)
- MMFA form (2 copies)
- Photocopy of health card
- Recent picture of the child
- Picture consent form (1 copy)
- Photocopy of ID (passport or birth certificate)
Fee Schedule for House League:
- $400.00 per player
